MEDAssist

PATHS' MEDAssist Program provides assistance to those in the Dan River Region who do not have prescription insurance coverage in accessing the most advanced in medication therapies.  many of the physician practices in the area participate.

PATHS' Community Dental Center

We are here to provide dental care to those local to the Dan River Region.  The links between oral health and overall health are well publicized.  In following its mission, PATHS wants to make sure that everyone who needs access to dental care gets it – regardless of their ability to pay.

All forms of insurance, including Medicaid, are accepted and sliding scale payment options are available for those who qualify. 

When someone with dental insurance or Medicaid chooses PATHS’ Community Dental Clinic for their dental needs, they are making it possible for someone without insurance to receive the same quality care.

Services Include:

  • Routine and Urgent Evaluations and Consultations;
  • Oral Cancer Screenings;
  • Digital X-rays;
  • Sealants;
  • Cleanings;
  • Periodontal Therapy;
  • Restorations;
  • Fillings;
  • Simple Extractions;
  • and new in 2011: Endodontics!

New Patient Information:

All new patients are required to complete a New Patient Registration Packet (CLICK HERE TO DOWNLOAD).  PLEASE NOTE:  Only those patients without dental insurance who wish to apply for the sliding fee discount are required to complete the part of the registration form that requests household financial information!

If you have questions while completing the form, you are encouraged to bring the form and all the necessary documentation (insurance OR income verification information, etc.) to PATHS Community Dental Clinic on any Friday between 8:30AM and 5:00PM where staff will be aviailable to personally answer all your questions and help to complete the form properly.

Once the form has been completed, you can either mail it to our office, or drop it by in person on any Friday during regular office hours.  When we receive it, we'll promptly contact you to set up an appointment.


Other Information:

Patients who have teeth extracted must take extra precautions to reduce the risk of complications or infections following the extraction.  We give each patient instructions when the procedures are done, but for added convenience, it's also available here.  Post Extraction Care Insructions (CLICK HERE TO DOWNLOAD).
