Job Description:

PURPOSE

Greets and assists patients by phone, and insures patients are scheduled correctly and efficiently. Maintains an efficient appointment and patient flow system. Works as part of a care team that practices evidenced based medicine in a Patient Centered Medical Home model of care.

QUALIFICATIONS

Education:      High School Diploma. Medical office experience preferred. Experience with medical office systems, medical terminology, customer service and medical software preferred.

         Other:    Knowledge of coordinating care with other agencies, computer experience, must be able to demonstrate excellent communications skills; both written and verbal. Must have diplomacy,

tact, and a positive approach to problem solving. Organizational skills, professionalism, along with confidentiality a must.

DUTIES

  1. Maintains incoming phone calls to the office daily.
  2. Assists patients with scheduling appointments for Follow up, Rescheduled, Cancelled, and No Showed.
  3. Checks encounter forms, medical records, and multiple schedules to prevent double or missed appointments.
  4. Maintains accurate and up-to-date appointment system.
  5. Creates telephone encounters for medication refills, nurse messages, and chart inquiries.
  6. Answers questions regarding patient accounts, referral appointments, and upcoming office appointments.
  7. Collects and updates all patient demographics. Advises patients when documentation is needed before their appointments. I.e. Proof of income for sliding scale, and current insurance information accurately and efficiently. Enters all appropriate data into the Electronic Medical Records system.
  8. Advises patients to sign medical record releases before appointments in order to receive outside medical records to insure proper medical care.
  9. Explains PATHS’ services, as well as patient policies and procedures.
  10. Functions within the HIPAA laws to protect patient information.
  11. Other duties as assigned.
