Works as part of the patient care team to assist providers and other clinical administrative staff in the provision of primary health care.  Under the direction of the Site Manager and the Clinical Manager, the LPN will embrace the standard and clinical protocols as set forth by PATHS.  Will provide professional, multi-disciplinary support to the providers as well as patients. Works as part of a care team that practices evidenced based medicine in a Patient Centered Medical Home model of care.

Education and/or experience

Current LPN licensure with sufficient experience to carry out the duties of this position and must be qualified in basic life support techniques.  A minimum of 2 years in a primary health or family practice setting preferred.

  • Ability to maintain quality control standards.

Essential Functions

  1. Takes patient vitals such as temperature, blood pressure, pulse and respirations.
  2. Collects information on the health status of the patient
  3. Participates in the development of the plan of care with other team members and safely implements that plan
  4. Communicates in an attentive and individualized manner in every encounter.
  5. Verifies the patients understanding of the purpose of visit.
  6. Provides instruction/teaching to the patient according to the educational plan.
  7. Documents care provided or procedures performed accurately on patient records.
  8. Provides each patient care in a safe, comfortable and private environment.
  9. Provides preventive care information that matches patient age, sex and health status.
  10. Assists as necessary in medical exams.
  11. Cleans rooms in between patients including wiping down beds.
  12. Draws labs as appropriate.
  13. Administers injections or gives medicine under the instruction of the provider.

Job Description:

This individual will work as part of a patient care team by assisting medical providers and other clinical administrative staff in the provision of outpatient family and/or pediatric medical care.  Under the direction of the Site Manager, the LPN will embrace standard and clinical protocols as set forth by PATHS.  It is expected that the individual will provide professional, multi-disciplinary support to providers, as well as patients.

Specifically, this position will:

  1. Communicate with patients and visitors in a courteous and professional manner, address questions and needs with sensitivity to patient confidentiality, cultural diversity and individualized requirements for assistance;
  2. Address patient concerns and assist patients with conflict  resolution and problem resolution;
  3. Take initiative and accept accountability as a patient advocate and promote patient satisfaction according to PATHS' mission and vision;
  4. Perform other necessary duties, as required, to meet the needs of PATHS' Providers as well as other departments to facilitate the best patient outcome;
  5. Perform duties that are outlined in clinical protocols and procedures, and accept Provider input/direction;
  6. Ensure an appropriate environment for the administration of healthcare by maintaining exam rooms, nurse’s station, equipment and other work areas in a clean, safe and orderly fashion;
  7. Assume personal responsibility for infection control and bloodborn pathogen precautions to protect patients, staff, and self from risk of exposure;
  8. Demonstrate enthusiasm and commitment toward continuous learning and will share knowledge and expertise with others to ensure the continuity of patienty of patient care;
  9. Record the patients’ vital signs (including blood pressure, pulse, and respiratory rate);
  10. Collect patient’s health information (i.e. reviewing medication lists, surgical history, etc.);
  11. Relay information between patients and other members of the care team;
  12. Give oral medications and injections to patients when ordered by Provider;
  13. Perform lab tests (glucose, A1c, etc.) and other tests (i.e., EKG) according to standing orders and as instructed by Provider; and
  14. Ensure proper, accurate and timely charting, documentation, and maintenance of patient’s medical records.

Preferred Skills:

Current LPN licensure (Virginia) with sufficient experience to carry out the duties of this position, which include certification in BLS (basic life support).  Ideally, the candidate will have a minimum of two (2) years experience working in outpatient family and/or pediatric medicine.
